Address Proof

​

There are two types of address proof to the applicant known as Direct and Indirect.

​

1) Direct Address Proof

​

It is the applicant's own address proof.  Following is a description on the valid address proof in term of source and type that accepts by Transport Department.

2) Indirect Address Proof

​

As the name "Indirect Address" implied, the place the applicant live is provided by some body, normally is the employer of the applicant.

It is a must to proof the applicant is working for the provider.  The employment contract is the best document to proof.  Then goes together with the Direct Address Proof of the provider, the employer, this address proof could indirectly serve as the address proof of the applicant.

The direct address proof of the employer as illustrated in the above diagram, it could be a bill from utility companies such as electricity, water, gas, rate, telephone or bank/ insurance statement etc.

Note:

​

The address proof cannot be an envelop.  It normally is the first page of the document that has following data printed:

​

a) Name of the addressee,

b) The address, and

c) The issue date of the document.

​

It is a must the current date has to be in the three months date range from the issue date.  For example, if the issue date is 1 September 2021, then say the current date 10 September 2021 is in the date range between 1 September 2021 and 30 December 2021 then this document is a valid address proof of the addressee.  Otherwise it is invalid.
